Eligibility Criteria for the Day Health Program

Adults eligible to become participants in the Dr. Peter Centre Day Health Program are those most at risk for deteriorating health.

Eligible participants must be diagnosed with HIV/AIDS and usually live with several of the following challenges:

  • Multiple health issues (mental health conditions, substance use, addiction, Hepatitis C, etc.)
  • Limited financial support
  • Limited or no family or other social support
  • Unstable physical health
  • Unstable housing situation
  • Physical disabilities.

Referral and Admission Process for the Day Health Program

Adults wishing to attend the Day Health Program may fill in an application form available at the Dr. Peter Centre West End reception. An admission assessment interview assists in determining if the Day Health Program meets the need of the individual. Admission is prioritized based on the degree of need. Those not meeting the eligibility criteria at the time of the application are encouraged to re-apply in the future should their health status change.

Eligibility Criteria for the Residence

Adults eligible to become residents in the Dr. Peter Centre West End Residence are individuals who require 24-hour nursing care. Generally residents have been unable to manage individual living, despite social and health care community assistance, and would benefit from 24-hour nursing care.
